What is a Patient PORTAL?

Patient PORTALS are FREE, secure, electronic sites that holds patient health information. Use of the portal offers convenient access to portions of your childs medical record. This is the best place to aquire your childs most recent physical and immunization record.

Through the portal, you can access:

  • Upcoming Appointments
  • Medical Conditions
  • Medication List
  • Lab & Test Results
  • Immunizations

The portal is also a convenient way to:

  • Request or change appointments
  • Request Medication Refills
  • Receive reminders
  • Download and complete various forms
  • Pay your bill

PCHP will NEVER sell your email to ANYONE nor send you junk mail!

Registration is easy. All you need to do is call the office for your access code or wait until your next visit and it will be provided to you at that time. Patients who are over the age of 14 may be the respresentative for the portal themselves or authorize a parent/guardian to be the representative for them. Patients at or above the age of 18, may request access to their own records via this patient portal.

Requests for Medical Records

The patient PORTAL is the most convenient way to access and print medical records. If one does not have access to a printer Plattsburgh Primary Care Pediatrics will print minimal information for a $5 fee. Cash only or we can fax the information to your new Primary Care office for free. Minimal information includes:

  • Last physical
  • Immunization list
  • Growth chart
  • Current and past medications
  • Allergy list
  • Problem list

Minimal information requires a 48 hour time frame to complete the request.  If the caregiver or patient would like a complete copy of all related health data, our medical records staff asks a 7-10 business day turnover time. The digital copy will be transferred to an encrypted CD.  A fee of $10 per child will apply. If the document will be mailed the recipient must have WinZip and Adobe to access the information encrypted.

Any information needing to be completed by the physician or nurses can be dropped of at any time with a contact number on file to get in touch when the documents are ready. There will be a minimal of 48 hours asked to fulfill the request.

Transfer of RECORDS : In order to transfer records IN to our practice (New Patient) or transferring records OUT of our practice (transferring medical care to another provider - pediatrician, adult or family medicine), you must complete a RECORD RELEASE.

PLATTSBURGH PRIMARY CARE PEDIATRICS CANNOT RECEIVE BY FAX OR SEND EMAIL CONTAINING PATIENT  INFORMATION.  THIS IS IN DIRECT COMPLIENCE WITH HIPPA.
